Why they get conflated
Good KPI design starts from the outcome and works backward; bad KPI design measures what's easy and assumes it correlates.
Why it matters
Without an explicit link to an outcome, KPI management becomes optimising a number without organisational effect.
How to tell them apart
Ask which outcome this KPI is assumed to predict, and whether that link has ever been tested.
