Delivered result or achieved effect.
The delivered result: the space is clean, the fault was cleared within four hours.
The organisational effect: operations did not stall, the inspection was passed, people could work.
The difference in one sentence
Output is what was delivered; outcome is the effect that matters.
Why they get conflated
Outcome-based contracts are marketed while the KPIs measure output. The language changes, the measurement does not.
Why it matters
A provider can hit every output while the organisational outcome deteriorates.
How to tell them apart
Ask whether the indicator would shift if the core process changed. If not, you are measuring output.
